Game crashes - blue screen of death

Since yesterday I've been having serious problems when I enter the game. Sometimes the game freezes, the music keeps repeating, and even my computer becomes extremely slow.
I noticed that when this happens my SSD has 100% uptime.
And sometimes I get the blue screen of death, saying memory management.

I don't know what else to do, as it is currently unplayable.

Settings:
CPU: AMD Ryzen 5 2600
RAM 16GB
GPU: Radeon RX 580

Some more info:
Sometimes the music stops but I still play without any problems. Other times it stops when entering the city. And sometimes it gives the blue screen of death when selecting the character. CPU, GPU and RAM are nowhere near 100% but the game remains frozen
