Nameless seer + pandemonium scarab spawn bug

Nameless seer didn't spawn for me 2 times already while using Horned scarab of pandemonium on Atoll and Strand. I've checked multiple times so there is no way i could have missed him. I think pack replaced by a boss doesn't count towards nameless seer therefore he cant spawn.
