Path of Exile 1 Gameplay Help and Discussion - Unable to unlock 5 slots in map device - Forum

Not sure if I'm doing something wrong but I've just ran 3 four ways in a row and it is still not giving me the fifth slot for my map device? Are there other requirements for this?
