No afflication league mechanic spawn in channel map

The affliction league mechanic is not spawning in the channel map, aqueducts in act 3 (entering from both ends), and blood aqueducts (also entering from both ends). I have talked to multiple people only one of them can reproduce it, I have a 100% success rate in reproducing it.
(it's not spawning late in the map I have done a full run-through multiple times and it's not spawning anywhere)
