How to get twitch icon

I'm seeing other streamers on the ladder with the twitch icon that links to their account, how do I add this? I'm not showing up on the poe ninja streamer ladder because of this.

I've been checking while streaming path of exile and it hasn't shown up, not sure what I did wrong.
