[SOS! Help needed!] Chieftain Hollow palm Vaal Ground Slam

I need a help from someone more experienced with melee/hollow palm builds, I've made this build and I would like some help on what I could've change to get it more tankier - more dps.

Currently sitting at 620k dps with 5k hp, Need massive help so if you have a minute, please tell me what am I missing there.

Plus, I'd love to hear what am I doing wrong in the setup - passives, I'm new to making builds on my own so it would REALLY HELP a LOT!
