To answer one of your questions, any build that cared about tankiness would be happy to have it. Physical damage is the hardest damage type to mitigate (with the possible exception of chaos damage). However, you'd have to be okay with not having an enchant, and not having any other good mods on your helm. Not to mention the cost of buying it.

Yep, it's a Fire res + phys to fire Inc mod + T1 Warlord. This actually can be divined to 15%.
