So the question is how to get/craft it?

-> boss might drop rare rings with temple mods on it
-> tier 3 sanctum of immortality (https://pathofexile.gamepedia.com/Incursion_room) might drop a magic item with a life mod on it

...those are slow though

2) blight chests with jewellery can drop temple mods on rings

3) metamorph encounter with "incursion jewellery" can drop temple mods
