WTB specific-affix ghastly eyes: T1 life, T1 damage, & fire/cold resists (or full damage suite)

looking to buy 2-3 Ghastly Eyes with the following affixes:

PREFIXES
1) 39+ life (T2 or T1 affix). While a high T2 is okay if everything else is present, T1 of 46+ is strongly preferred.

2) T1 Physical Damage (Minions deal 15-17 to 22-26 added physical damage).


SUFFIXES
3) Either of
* +4-6% minion attack and cast speed, OR
* +15-20% minion damage if you've used a minion skill recently.

4) Fire and Cold resist. All-resists is also acceptable, and I'll consider fire-only or cold only, but fire & cold is preferred.


I'm also potentially interested in 1-2 similar jewels with no resists, but both damage suffixes. Will consider 3-affix jewels with both prefixes and a damage suffix for slamming if life is T1.
