Delirium No Longer Completed After Death to Boss

I was in the Decay map right next to The Withered Willow, which I went all the way through (including activating the mirror). The boss was all I had left, and he got a good one-shot in and killed me. Upon reviving, I killed the boss, then left the map. It would not let me traverse to The Withered Willow. I realized the node of the map I just ran was orange, not green.

I went back into the map, and it now says "Activate the Delirium Mirror" again in order to complete the map. It does not exist because I already ran it. I even went through the entire map again just to make sure it did not respawn, which it did not. The map is now unable to be completed, unless I run it again, which I now can no longer use tablets on nor does it have other league mechanics in it because it thinks I failed it previously.

I was able to get into the map again, so I tried to replicate it. Same thing happened when I died to the boss again. Reference ID: 2,304,352,422

I have the same issue cant progress the quest and now cant anoint amulets due to this
