EDIT: My mistake - it was hiding in the Verisium Anvil. I must have forgotten to take it out after checking the upgrade stats. Sorry!
This character (Huntress) had the Unique spear Tangletongue equipped on most recent logout, but when I logged back into the character, the main hand slot was empty. Possibly related: While playing another character (Witch), the same weapon dropped, and I stashed it, intending to compare stats when I next played the Huntress.
