Ruins of Kingsmarch - lost Triskelion

After repairing the Triskelion and going to the crater, putting the Triskelion into the slot to activate the boss fight did nothing, and the Triskelion item disappeared. Unable to get the item back or restart the instance to initiate the encounter.
