Cannot complete map

I did a Decay map. Completed all other objectives on map first. Died against boss. On reload it placed me somewhere else on the map at random, and upon defeating the boss the map didn't complete, instead demanding me to "Activate the Delirium Mirror", which was the first thing I did in the map. Upon exploring the entire map again, no new mirror was discovered.





Anywho, hopefully those images embed showing it off. Else I'll have to link me bsky for it.

Dying to a map boss removes all other content in the map except the boss.
