Bug Report - The Origin Tower - Bricked End Game State

Was in friends party, we both completed the Patriarch and Matriarch Halls, collecting our 2 pieces. Host inserted those 2 pieces in to "The Origin Engine", I did not as they clicked through the dialogue faster than me. My quest was updated to their stage. We went up the lift, host inserted the item to spawn the boss. We killed the boss, my quest finished. No rewards or change of my atlas.

I still had the 2 Hall pieces, went in to my own instance and inserted them in "The Origin Engine". I collected the item to spawn the boss, I was unable to kill it before heading to bed for the night.

Logged in now, I have no Hall pieces in my inventory. I am able ascend the Origin Tower, there is no boss at top. I can enter the Engine, it prompts me for the 2 pieces from the Patriarch and Matriarch Halls.

The 2 Hall's are green and unable to be completed again.

I have this exact same issue, please help us!

I even farmed a set of the open work keys and it wont let me put them in.
