Immortal proximal tangibility hiveborn during hive fortress

Trying to complete the quest hive fortress and I guess one of ailiths skills killed a proximal tangibility hiveborn offscreen. It then continued to exist as a immortal ghost hiveborn that continued to attack and do damage which eventually resulted in losing the hive fortress encounter because it aggrod on ailith.
