Stuck in Twisted Domain after defeating Xesht (PS5)

After defeating Xesht in the Twisted Domain, I became unable to leave the area. The portal does not work, and even after restarting the game, my character still starts inside the Twisted Domain.

I am completely stuck and cannot continue playing this character.
