Dying in a map sometimes bricks the map

Occasionally, when I die in a map, there's a popup that flashes for a second and then disappears.
When I respawn, the map is completely empty and all the league mechanics have disappeared. The map will completely reset, the minimap is hidden and all the checkpoints are now hidden. I can kill the boss, but do not receive credit for completing the map.
