Expedition Tomb not spawning mini boss or spawning exit

After entering a tomb, clicking the verisum ore did not spawn the boss and the exit is not active to get back to the map again.
Hade to TP out of the map and abandon it.
Before entering the tomb and encountered the bug I TP to hideout - if the information is good to recreate the bug.
