Missing Medved's Crest of the Circle

I have finished the campaign and went to upgrade a unique weapon. The Verisium Anvil tells me I need a Medved's Crest of the Circle, which I do not have.

Google (https://maxroll.gg/poe2/resources/campaign-league-mechanic-rewards-cheat-sheet) tells me this should be from the rune encounter in the Drowned City, which I have cleared and found nothing.

All my Act1-3 quest are showing as completed, including The Runeseeker in each act. Only Act quest not completed is in Act 4: Fate of the Vaal (Enter Atziri's Temple).

After reaching maps, I migrated from SSF. Is this related?

What do I need to do?

I bought the crests I needed from currency exchange, but still wonder what happened.
