My burning monolith is bugged

I killed the boss touched that little esphere and talked to that guy after. a portal opened down and it sent me to the first act city.

When I tried to go in again it was asking for the 3 crisis fragments and the npc shows me the bosses I killed to unlock the monolith before but now without even the first crisis fragment which we get when we get to the burning monolith, Idk what to do I'm probably stuck now
