Game crash during finding grand mirror

Each time I'm doing a tower and press the precursor table thing for the tablet and complete the map I get a n otification that the delirium is spreading and that another grand mirror has been discovered.

Each time the game crashes and I lose the instance, everything I have in my bag etc. I did notice that the new amulet from "loathless mire" did not get removed even thought it was from the same instance, maybe because it was an instance inside the instance so it got saved?

I have completed all the delirium points and done the introduction quest. I have a grand mirror somewhere already so I'm not sure if it crashes if there are multiple or because it's trying to spawn on another mechanic and then fail.

I am getting this crash as well. Once the boss is killed, I see the banner for the grand mirror and then the instance crashes.
