t15 map essentially deleted after dying

i died in a t15 reservoir map to the boss. map had revives available

after respawning and killing the boss, nearly every mob in the map had disappeared (i found one but it said 19 monsters remain), all league mechanics in the map were gone, and my map overlay progress had been reset
