ANCIENT BEACONS

Find and activate an Ancient Beacon - i alsow completed all beacons in the map but the quest is still available what should i do?

Same problem. For me the vaal ruins portal spawned after I completed the 6th one but I forgot about it and the instance ended up resetting. Now every one I go back to doesnt spawn the portal. Theres a reddit thead about this where someone mentioned another encounter in the interludes. I guess thats the only solution right now.
