Lost Map Objective (physical, not quest) on Respawn

I died after killing the boss and my map became unexplored. I still had one map objective left (energise vaal beacons) so I re-explored the entire map. The rare along with all the other monsters in the map were gone. It was an empty map with no vaal beacon to energise. Just to make sure I didn't miss a location, I made sure every white outline on the of the map was showing.
