EVENTIDE PETALS + HOLLOW FORM SWIRLING ASSAULT INSTANT CRASH

When i hit a mob with a hollow form swirling assault hit the game instantly crashes, ive been able to reproduce the issue in several instances, along act areas and maps, the item simply cant be used with this skill combo.
