Breach atlas tree shows 32/32 but last point cannot be allocated / no breachstone indicator

My Breach atlas tree shows 32/32 completed — I've finished every map in the area but there's still one passive that looks allocatable yet clicking it does nothing

Second problem: i converted 300 Breach Splinters into a Breachstone via the Genesis tree, and the Breachstone is now sitting in my inventory (not stash). But I have no way to actually use it on atlas. There's no indicator anywhere on the Atlas showing where a Breachstone is supposed to be activated
