Rite Of The Nameless Bug - Incompletable Map Objective If You Die

Had this bug happen twice. If you die DURING a Ritual in a Rite Of The Nameless chain, the map objective changes from "Complete All Rituals" and "Kill The Boss" to "Kill All Rares". Even if you kill all the rares, or the rares are all already dead, the objective never completes and the map is impossible to finish.
