Tier 2 Map Bug

Tier 2 map is bugged i think. I accidentally died to boss 1 time then the second time I beat it but the map is still in progress with quest saying I have to clear all unstable breaches. Checked but since I died once the map is empty and it's saying that the map is in progress on the atlas map.
