Frequent crashes using Hollow Form with the new Azmeri Weapon Effect applied

I've crashed 7 or 8 times in Jiquani's Machinarium, about 20% of the way through the zone. The crash happens immediately after using Hollow Form, approximately every 10th use or so, and seemingly when it consumes power charges targeting an enemy near a wall/obstruction or in a corner.

I know you guys are busy just after league launch, so I did some testing myself and found that disabling my weapon effect (the new Azmeri Weapon Effect at Tier IV) stopped the crashes entirely, so far at least.

After disabling both my weapon skin (Stormborne Weapon Skin) and the Azmeri Weapon Effect, I was able to finish the zone without crashing. In the next zone (Jiquani's Sanctum), I re-applied the weapon skin without the weapon effect, and continued for a little bit without crashing. I then re-applied the Azmeri Weapon Effect Tier IV, and crashed after clearing a few more packs (Bug report reference 2471403268 for this instance). I re-entered the game and continued with the weapon effect and weapon skin applied, and replicated the crash once again. Finally, I removed the Azmeri Weapon Effect while keeping the weapon skin applied, and finished the remaining ~80% of the zone without crashing (bug report reference 1761992027 if needed, although it was generated in the same instance as the previous reference number).

It may also be worth noting that I had just allocated Hollow Form and began using it in Jiquani's Machinarium, and that the first crash occurred before 0.5.0b Hotfix 8 was applied. After the 1st crash, Steam triggered the update (which I see fixed a client crash related to Hollow Palm), but I crashed about a dozen more times even after the patch was applied.
