Disconnected Reason Unknown

Sometimes I get booted to Log In screen after I kill the map boss and it rolls back my map and inventory while saying I attempted the map.

It rarely happens but I think the cause is my abyss quest is bugged. When it tries to do the 5% chance to make a nearby map over run with abyss it fails, considering I run abyss every map and haven't seen it proc once.
