League Mechanic quest objectives uncompletable

- Map is one of the early questline breach maps
- entered, completed the breach
- died on the boss
- re-enter and there are no monsters in the area, and the map objective to 'open all unstable breaches' is again active, but there is no way to complete it as no breaches remain
