Breach Mechanics buggy as hell

Bro the breach mechanics is buggy af on console. If you die while doing a breach it respawn you in a new instance of the map, which has no breach. So the map is counted as not completed. Happened 3 times already. Basically you have to do breach map no death or you're fucked.

I cant even get through 3 maps this after because it keeps putting me in new instances. This is so frustrating, I still have 2-3 revive but all breach maps is played like max waystone modifiers with 0 revive.
