Abyss strongbox not completing

I have ran several abyss strongboxes and they will not complete. they continue to shoot out which ever mechanic they are (IE the chaos orb circles, the ice spikes from the gound). Not sure if a monster is spawning in the ground or what the case is, but im losing out on loot from the strong boxes.
