Softlocked Abyss

Was progressing through Abyss, died on the boss before completing Abyss on the map and this activity completely disappeared. Now this map cannot be completed. Tried to use new waypoint on same area, the activity is still missing and cannot be completed. First image shows the map state if I try to open it with the waystone (no Abyss activity, but I need it for completion). Second image shows that for progressing through Abyss I need to complete this map first or I'm stuck, basically locked and cannot progress through Abyss campaign. Last image shows the full cleared map of the area with no Abyss and boss killed.

It happened after I died on a boss, new map generated without Abyss, but map completion status says "Complete all Abyss"
Any new map instances don't contain abyss anymore, it's blocking my quest

Specifically I died at the forge before even getting to the Abyss Hub. Now I'm locked out because there league mechanics in a failed map location, and I need the Abyss to complete the map.
