ACT 2 The runseeker cant complete

went into vault in Mystic Refuge talked to the npc waited for hime to finish is dialog. when it popped up for me to "Study the Runic Knowledge" the book did not appear on the diplay in front of the pool. this is my second character through and it worked on my first. now i cant acess options during the random rune crafting events

tried creating a new instance but now the is no option for the NPC to spawn in.

same for me cant continue past mystic refuge.
