Can't get last/4th frayed/torn map piece in act4

My inventory has 3 unmovable map pieces and I can't get the last one since my quest map already says I've gotten all 4. I even checked those areas and the quest map pieces don't spawn...

same here, all map icons greyed out, places are not interactable, only 3 pieces in inventory

You can turn these in to Makoru. You don't need all 4 as you can turn them in 1-by-1. See if turning them in to her can fix your issue?

yep, turning in 3 of those after beating act 4 final boss fixed the issue
