Soft Lock. Mystic refuge

I died before speaking to Farrow in mystic refuge and now the quest state is 'speak to Farrow' but he is not there even if i reset the instance or reboot the game.

Check your quest log! You can find the door before clearing all 3 rune landmarks, but it won't open until you've completed the specific objective for each area. I was stuck until I realized I hadn't finished the Aztec Bog event yet. Once that zone was cleared, the door opened.
