One the monsters was bugged out of range, would not attack or move and could not be damaged. Reset the instance and now the map is empty and I cant complete the quest.

Same thing happened to me with the quest lock, but I died to a rare with a mana sucking circle that was misaligned so I couldn't see properly where it actually was.
