Several of the same error: In 3 maps the Mirage (portal) did not spawn, can't go to kingsmarch, and from kingsmarch it won't let me go to hideout several times over the last hour. all: Failed to join any instance...

One time the game just closed/stopped when I tried traveling to Kingsmarch from the waypoint shortcut button.
