bricked saresh fight

So i managed to brick two saresh fights somehow it looked like he died during the big slam he does and he just vanished and didnt drop anything i would guess it happened becouse slayers headsmen node but i cant be sure.
i killed him around 400 times and it only happened twice so not the end of the world.
Did it happen to anyone else?
