Scarab of Influences does not always spawn bosses

Using the scarab of Influences on maps from the “Influenced” series doesn't always trigger the appearance of additional bosses. I've noticed this on T16 maps from the “Influenced” series.
Additionally, on maps from the “Shapers” series, sometimes only one boss appears in the first phase, but if it's defeated, the loot disappears between phases.
