Strongboxes with the "of Singularity" mod sometimes do not drop unique items.
Same goes for Kaom's Cache. I have encountered this several times in Mirage SC. Today, it happened with the Vaal side area "of Dark Blessings" mod. And I managed to capture a screenshot & bug ID.  A sword he brought, his foes to maim and rend, from places dark behind forbidden doors, But night by night he woke with frighten'd roars from darkest dreams, too strange to comprehend.
