Trouble picking up items from the ground

When I do something that drops a lot of items like Breach I have a very hard time picking up items. Even if I have the item highlighted to pick up I can't pick it up. I assume it's because I'm too far because sometimes if I just move around it will let me but many times no matter what I do I can't pick certain items up.
I'm playing with controller so not sure if that has any bearing on this but jesus this is annoying. Does anyone have any tips or a workaround or some way to increase the pick up radius or something? Maybe I just need to switch a super strict loot filter or something but it's already on semi-strict. *edit* to add that if I leave the instance and come back it will frequently consolidate the items again and I have no issues at that point but this is not a great workaround. " If you're picking up few items then yes, all you need is a stricter filter, you can also edit your own to hide trash loot (gears below 79, shards n stuff). I'm using mnt filters from strict to uber from campaign to maps, it does really show only what you need.
