Disconnects on Warden version of Kinetic Fusillade

I keep experiencing disconnects while changing phase of Elder Guards fights. It seems not to be related to a map, the guardian itself or whether or not I have Maven watching them. But reading all the forum, I noticed that almost everyone who reports abnormal disconnects with instance crash and inability to connect back to it, plays (or has played at the moment) this specific version of Lolcohol's Warden Kinetic Fusillade. I thought it might be Tornado with Spellsinger, but apparently not, tried removing tornado, disconnects still appeared. I also tried not to use tincture, sigil of power, disconnects still happen. This has to be something about the skill specifically, self-shot version since Hierophant with Ballista setup don't seem to be affected. Went to kill Maven. One death and I can't re-enter fight, it gets disconnected every time

Added in game bug with as much info as I could right before disconnect, outside of boss arena

I am getting this same issue with Warden Kinetic Fusillade.
If I die to any monster that is tanky, I cannot go near the area where they are. I will disconnect immediately. I have also had disconnects where it gets very laggy (but I have not died). Same exact issue with changing phases in Elder Guardian fights. Means I cannot progress on elder without buying fragments and I cannot acquire a Maven Twisted Invitation. The same issue also occurs after any death during a boss fight; trying to re-enter portals leads to disconnection (while consuming the portal). So all non phase changing bosses for me are one attempt only, while phase changing bosses cannot be completed.

I ended up rerolling away from KF because GGG doesn't even seem to try to fix it/communicate back that they're looking into it.
