Lycia in Forbidden Sanctum does not go to final arena - second phase bugged

Lycia 2nd phase (of 3) in the Forbidden Sanctum bugged out on me at apparently 0 HP, she dashed a few times just as she was about to phase, dashed to me at the bottom right edge of the arena and vanished. No portal to the final arena and no visuals and voice of her transforming, no wings, just gone. There may have been a slight stutter of the game engine when the missing effects were supposed to happen, this would have just been a slight feeling of a lack of responsiveness for a fraction of a second. Could not enter the rewards room, had no way of getting to the final arena. Red mist still on the ground from the first phase.

When I fought first phase, I ran through the last part of the red mist to the 2nd phase arena without killing first phase, but had still taken a chunk out of her health bar by that point. This is the stretch where we're dodging fireballs and lasers.

As I'm writing this report my game logged out in the background and can't log back in, it's a 2-3 minute internet outage on my end. This starting about 5 minutes after the inability to complete sanctum. I suppose this could have just been terrible timing on my end and my local node network getting a late-night reset. Maybe a few initial missing packets caused the incomplete. It didn't feel that way though. It didn't log me out of sanctum, I was able to run in circles for several minutes, generate a bug report and then sign in to the poe site before my internet cut out, for instance.

When I logged back in I was still able to attempt to complete the entire lycia fight again from the beginning, which is a great feature.
