The King in the Mists encounter softlock

It is possible to softlock the encounter with King in the Mists when the fight transitions from phase 1 (when the boss reaches 25% health) and teleports you to the maze if the boss is standing anywhere on one of the three platforms when teleporting you away.

Even if the boss is standing just on the edge of the platform when teleporting you away, after you go through the maze the boss is unresponsive and does not transition into phase 2.
