instance crashing upon join, 'unexpected disconnection has occurred'

During content that has heavy load (Delirium, Blight, some Maven witnesses) the instance will crash & throw 'an unexpected disconnection has occurred'. The instance will remain unjoinable and continuously throw the same error every time you try to rejoin.
