2+ delirium bosses with delirium scarab of neuroses

Often using delirium scarab of neuroses I encounter 3 bosses instead of 2. From my expirience its always omniphobia, and the issue is replicable in Mirrages as well. Due to the scarab giving +1 when killing bosses is it an exploit?
