Foulgrasp Support potentially bugged

I have been playing a build based around the foulgrasp support but have found a substantial number of enemies seem to not proc the hands to explode. Initially I thought it was just stationary enemies and while this is mostly the case there are also some moving enemies which also do not proc them, realising this I assumed it might be either a matter of over capping, there by replacing, the hands before they could explode or something with AoE but having tested adjusting variables with both this seems not to be the case.
