Seconded. It occurs occasionally, maybe once in 50 fights. I think it happens around when he's going into his immunity phase. He just disappears and the map becomes impossible to complete. Leaving and re-entering does not respawn him. He drops no loot PoE Major achievements: 8/28/2016 - I chanced a Headhunter in a 144 leather belt chancing session

I had this happen twice, and i think it happens if u don't leave middle platform while he swirlys into middle. I was on top of the stairs he dived and never returned
